We work with local health providers to ensure greater service and system integration between primary and acute care. Our region aligns with the two Local Health Districts – Illawarra Shoalhaven and Southern NSW.
We partner with our Local Health Districts (LHDs) to enable all people living across the region to access health services that respond to their life circumstances; are coordinated, safe, high quality, accessible, timely and efficient. We work collaboratively on the below initiatives:
HealthPathways is a free web-based portal to support health professionals in planning patient care through primary and secondary health care systems within the region.
We support HealthPathways to document clinical referral pathways. Our region has two different HealthPathways resources: Southern NSW and Illawarra Shoalhaven.
Our Framework was designed and developed with Southern NSW and Illawarra Shoalhaven LHDs, community-based organisations, and people with lived experience of mental illness.
It ensures people with experience of mental illness continue to shape and deliver mental health services.
We worked with the Illawarra Shoalhaven and Southern NSW LHDs to develop a Regional Mental Health and Suicide Prevention Plan.
It involves working together across the health system, with consumers and other stakeholders, to provide better integrated care for people with mental illness.
We work in partnership with Southern NSW and Illawarra Shoalhaven LHDs to deliver exceptional care, and support community resilience following the bushfires.
In 2020, we developed a monthly eNewsletter which provided high-level updates about bushfire recovery and community rebuilding in the region.
Lumos is a partnership initiative between COORDINARE, Southern NSW LHD, Illawarra Shoalhaven LHD, and the NSW Ministry of Health.
The program generates new insights to improve health services and map patient journeys across the NSW health system.
The Southern NSW Integrated Care Strategy outlines our commitment to work with Southern NSW LHD to integrate healthcare.
Integrated care is a collaborative approach to strengthen person-centred health care delivery through the provision of coordinated, high quality care across the lifespan.
We jointly established four GPLOs with the Southern NSW LHD and one with the Illawarra Shoalhaven LHD.
This is a collaborative approach to improve healthcare providers’ awareness of available services across care settings, enhance communication, and identify challenges and opportunities to improve system functionality.
We trialled modes of care such as the Geriatrician in the Practice program in the Illawarra Shoalhaven, and chronic pain management in Southern NSW.

To ensure care is seamless, timely and appropriate, we have built a strategic alliance with the Illawarra Shoalhaven LHD.
